Statement of Thesis/Dissertation Approval

The Statement of Approval replaces the signature sheets in the thesis or dissertation. Copy and paste the form into your manuscript as page ii (after the copyright page), then complete the required information.

Do not scan the provided form or your signed approval forms. No signatures are necessary for the Statement of Approval. OnBase collects the signatures after submission.

Permission to Quote Copyrighted Material

When is copyright permission needed?

Most publishers have their own policies and procedures for granting copyright permission requests. We will accept whatever form of permission you are given. This includes:

  • a link to a journal’s website stating that authors’ retain copyright
  • Open Access information
  • an email reply from the copyright holder

You can also look for permission at the Copyright Clearance Center.

If none of these are available, send the appropriate form below to the owner of a copyright. Submit this form for every published article you include in your manuscript that does not have another form of permission.

FERPA Authorization

Due to FERPA restrictions, only the author(s), or those they authorize, may pick up an edited manuscript.

Submit the FERPA Authorization Form to the Thesis Office if you want another person to pick up your manuscript. You must submit the form before they arrive for pick up.
